Membership Mandate
This form is for creators and publishers who wish to join ACPOSL. By completing it, you become a member and authorize the ACPOSL to manage your rights on your behalf.
Instructions:
- Please complete all sections clearly and accurately.
- Attach additional sheets if you have more works than the space provided.
- Ensure that all works listed are your original contributions or works you hold rights to.
- Sign and date the form before submission.
Application Form for Public Performance License
This form is for event organizers, promoters, or venue managers who wish to obtain a license to publicly perform copyrighted music at their events.
Instructions:
- Please submit the form at least one week before the event date.
- Provide a complete and accurate list of all songs that will be used at the event.
- Once reviewed, an invoice will be issued based on the music usage. The public performance license is granted upon full payment of the invoiced amount.
- Be transparent and accurate to avoid any complications.
Split Sheet
This form is an agreement among collaborators (lyricists, composers, performers, producers, etc.) that defines the ownership percentage or “split” of each person’s contribution to a work. It ensures clarity and fairness when royalties are distributed.
Instructions:
- All contributors should discuss and agree on splits before signing.
- Percentages must total 100%.
- Each contributor should retain a signed copy of the agreement.
- Submit a copy to ACPOSL to ensure correct royalty allocation.
